Typically, your organization will receive a unique signup link to a HIPAA-compliant form for employees to fill out. If you can get them to fill it out before the day of, you’ll help ensure a smooth and efficient vaccination day.
Keep in mind that employees will need also to sign a consent form dated for the day of the vaccination, which can be completed in advance as well, thankfully. And, let your team know that they may also need to provide their physical insurance card and/or the last four digits of their Social Security number.
